Job summary

NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

Main duties of the job

NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced registered nurse with qualifications in LD, MH or General nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management. You will have knowledge of the National Framework for Continuing Healthcare (Oct 2018) and be able to:

o Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes, other providers etc.

o Undertake high quality holistic assessments of patients, clearly identifying needs using assessment tools as required.

o Manage a caseload of complex cases.

o Cover as Duty Nurse within the Continuing Healthcare Service.

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential.

A current driving licence and vehicle used for business use will be required.

NHS Frimley Integrated Care Board was formed on 1 July 2022 under the Health and Care Act 2022, replacing NHS Frimley Clinical Commissioning Group.
