Practice Pharmacist - Halton Place NHS Cheshire and Merseyside
Band 7
Two positions over 52.5 hours (1.4WTE) - Full or Part time considered.
The Medicines Management Team within Halton Place are looking to recruit practice pharmacists to provide medicines management support including delivery of the medicines management work plan across Halton.
Main duties of the job
This role directly supports the Senior Pharmacist and Senior Medicines Optimisation Lead with regards to safe, high quality and cost-effective prescribing including delivery of the Medicines QIPP plan.
The applicant must be a registered GPhC pharmacist with the ability to deliver medicines management and optimisation services in GP practices. You will work with our team of medicines management pharmacists and technicians to provide medicines optimisation support to GP practices across Halton.
You should demonstrate excellent clinical skills and must be computer literate. You must be organised, be able to work independently and also be an excellent team player.

The postholder will support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines. This includes patient medication reviews, helping practices achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care.
The postholder will act as a specialist resource to GP Practices, to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals, including secondary care when required.
The postholder will contribute to projects across the ICB footprint, as required by the Senior Medicines Optimisation Lead.
