JOB SUMMARY / PURPOSE
An opportunity has arisen for an enthusiastic and flexible Reviewer to join a new LeDeR (Learning from lives and deaths - People with a learning disability and autistic people) team supporting the combined Cheshire and Merseyside and Greater Manchester Team.
As part of a combined LeDeR workforce model across Cheshire and Merseyside and Greater Manchester the post holder will be part of a small, dedicated team responsible for completion of reviews following the death of people with a learning disability and/or autism in line with the new LeDeR - Learning from lives and deaths - People with a learning disability and autistic people (LeDeR) policy 2021.
The LeDeR programme is a national initiative established by NHS England in 2015 to review deaths of people with learning disabilities, learn from those deaths and take forward the learning into service improvement. The NHS Long Term Plan makes a strong commitment to reducing the premature mortality of people with a learning disability as well as improves standards and quality of care experienced by patients and their families.
Main duties of the job
- Educated to degree level or equivalent relevant specialist experience
- Registered Health or Care Professional (with active registration, without restrictions)
- Experience of working within health/care settings and multi-agency working across organisations
- Highly developed level of specialist knowledge of the complex health and social care needs of people with learning disabilities and/or autism and their application to wellbeing

- Highly refined analytical skills in order to make informed judgements and conclusions related to mortality reviews (Clinical outcomes, nursing assessments, complexity of health and care needs, quality of patient journey)

About us
To comply with the National LeDeR Policy published March 2021, from the 1st July 2022, responsibility for ensuring the delivery of LeDeR reviewstransferred to Integrated Care Boards (ICBs) from Clinical Commissioning Groups (CCGs). Local ICBs are responsible for ensuring that LeDeRreviews are completed for their local area. For Cheshire and Merseyside and Greater Manchester a combined dedicated reviewer workforce hasbeen agreed and commissioned which is being hosted by Cheshire and Merseyside ICB.The new combined LeDeR team has been fully operational from the 01st September 2022.The Learning Disability Mortality Review (LeDeR) programme is a national initiative established by NHS England in 2015 to review deaths ofpeople with learning disabilities, learn from those deaths and take forward the learning into service improvement.The aims of the programme are achieved by:o Delivering local service improvement, learning from LeDeR reviews about good quality care and areas requiring improvement.o Driving local service improvements based on themes emerging from LeDeR reviews at a regional and national level.o Influencing national service improvements via actions that respond to themes commonly arising from analysis of LeDeR reviews.
The post holder will be required to complete the relevant LeDeR training initially and then complete the refresher training on an annual basis.

Effectively liaise and collaborate with all key partners and stakeholders at all levels including the Local Area Contact, Senior Reviewer, LeDeR Administrator and other reviewers within the team. Conduct mortality reviews to a high level and attend local governance panels, update on the status of reviews, present areas of good practice and concern from each review and contribute to the discussion on determining actions to be taken and implement local service improvements as agreed.
- To support the Senior LeDeR Reviewer to deliver an ambitious programme of improvement, focusing on quality and performance in line with the NHSE guidelines and promote positive change throughout health and social care, based on the learning from the reviews.
- To provide a supportive and empathetic approach to the reviews and work closely with other professionals and families when reviewing each case.
- This role is integral to support the reduction on health inequalities.
Principal Duties
- To complete high quality reviews efficiently and within national timescale requirements and where necessary co-ordinate multi-agency reviews which may be of a complex nature
- Develop resources to support Reviewers to conduct quality reviews and enable Reviewers to adequately support bereaved carers and families with whom they come into contact through the course of undertaking a review
- To conduct both initial and focussed reviews, working collaboratively with the review team and other partners. LeDeR reviewers are expected to work collegiately and provide peer support to other reviewers, particularly around their own area of expertise and supporting progression to completion
- Ensure that Safeguarding principles are considered and applied within all elements of the LeDeR programme and establish links with key local Safeguarding contacts and ensure that any concerns are reported as per Safeguarding Children and Adults policies.
- Support the development of local LeDeR arrangements, working alongside the Local Area Contact and Senior Reviewer to ensure that the requirements of the new LeDeR policy are applied and embedded.
- Identify the most appropriate evidence available including the conversation with the family or loved one, conversations with the GP or GP records and other health and care data sources and using high level professional judgement and experience to determine and agree with senior reviewer, whether a review proceeds to a focused review.
- Explain the parameters of the LeDeR review, signpost to other processes such as complaints processes, and discuss with the family their expected outcome of any LeDeR review setting realistic expectations at the start.
- Present review findings at governance panels and actively participate and support the panel to define SMART actions offering professional challenge and proposing solutions to address recommendations identified in completed LeDeR reviews, to ensure that the actions that are developed will achieve the needs of the findings of the review.
- Agree with LACs (and senior reviewers where necessary) any actions coming out of initial reviews (these will be limited and infrequent).
- Share, present and submit the learning from completed reviews with the local teams and other partners to support identification of learning that can be actioned. To collate and analyse information from completed reviews and feedback to local groups and other forums as required.
- Have the ability to convey and present complex, sensitive and possibly contentious information to a range of audiences - this information includes, but is not limited to, feedback to and from families of deceased clients as well as learning and feedback from reviews which may challenge system partners and local groups.

- Lead a number of Learning Disability Mortality (LeDeR) reviews. In so doing communicate sensitively to relatives and carers regarding the life of and circumstances leading up to the death of a loved one or service user with a learning disability.
