Job summary

An exciting opportunity has arisen for a dynamic and self-motivated individual to join the Wirral Place ICB Transformation and Partnerships Team on a permanent basis.

Working in a demanding environment, the post holder will lead delivery of discrete project areas and support the Partnerships and Transformation Team to deliver transformation and improvement projects in areas include Mental Health, Planned Care, Primary Care and Urgent Care.

The successful candidate will be a strong collaborator, negotiator and facilitator, who is confident working in partnership with a wide range of stakeholders in order to achieve transformation of complex services.

Project Management and Business Planning skills will be fundamental within this role.

Main duties of the job

Responsible for supporting the business planning function of the ICB at Wirral Place,

Support the transformation of the planned care workstream, working with providers and the partnerships and transformation team to deliver improvements in pathways for maternity services, cardiovascular disease services (CVD) over the coming months.

The post holder will support the redesign of CVD Prevention services in line with the Cheshire and Merseyside CVD Prevention strategy, working with Primary Care and third sector agencies on Wirral.

The post holder will also be responsible for supporting the inequalities work as well as other key transformational work programmes, as they arise.

Main duties of the job

Be a core member of the Partnerships and Transformation team and to support the Partnerships and Transformation Manager in ensuring the transformation of services across Wirral Place.

Lead on discreet areas of project work and report back progress to the Partnerships and Transformation team meeting, managing competing priorities and completing work to agreed timelines.

In conjunction with the relevant Clinical Leads, the post holder will be required to support Transformation for an agreed section of the transformation portfolio; ensuring effective collaborative working with internal and external partners as required.

Working with the relevant clinical lead, ensure the development and implementation of transformation plans for the specific portfolio of services for Wirral Place ICB and wider Wirral Health and Care system.

The post holder will work across Mental Health, Planned Care, Primary Care and Urgent Care.

The post holder will develop and maintain strong working relationships with multiple teams, across the Wirral Health & Care system.

The post holder will be required to represent Wirral Place in and contribute to various programmes of work within the portfolio.The post holder will need to demonstrate the skills required to manage the complexity of the work and working relationships with diverse stakeholders.

Transformation for the identified portfolio:

  1. Manage internal and external relationships to promote effective transformation and commissioning arrangements, including, where relevant opportunities to undertake collaborative or aligned transformation and commissioning with wider partners.
  2. Promote clinical input to, and innovation within, the transformation and commissioning process.
  3. Work with the clinical leads for the relevant portfolio area to transform and commission high quality services within primary, community and secondary care as appropriate. Linking with wider commissioners, partners, providers and stakeholders as appropriate.
  4. Identify and implement service re-design opportunities which support the overall strategies for the portfolio of services.
  5. Ensure appropriate patient and public engagement.
  6. Foster effective working between the ICB Clinical Leads and commissioning staff.
  7. Be responsible for transforming and commissioning high quality services for the residents of Wirral Place, taking into account assessment of local health need, local plans and priorities, national policy and financial priority, and overseeing the work of staff who undertake work for them.
  8. Be responsible for ensuring the ICB meets national policy and achieves the relevant targets in the portfolio areas.
  9. Assist in the development of the ICBs Strategic and Operational Plans, Commissioning Intentions and support the Senior Managers in fulfilling the overall plans of the ICB.
