Job summary
The post holder will work in the Children and Young People's Continuing Care Team, undertaking assessments and reviews to assess whether a child's needs are such that they meet the criteria for full NHS continuing care. The post holder will be expected to undertake NHS Continuing Care assessments, comprehensive nursing assessments, contribute to the completion of EHC plans and support during multidisciplinary team meetings when completing Continuing Care documentation. The post holder will provide professional, clinical, and subject matter expertise on matters relating to children's Continuing Care processes for the ICB, Local Authority, healthcare providers and other local agencies and organisations.

Main duties of the job
The post holder will be responsible for undertaking assessments and reviews of individual's eligibility for Continuing Care and case manage existing patients. The post holder will ensure that assessments and reviews are carried out in a timely manner according to national specifications.
