Job summary
This role will lead and manage a team of assessors who administer and undertake assessment of the patient's eligibility for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) in order to plan and commission appropriate care packages to meet their needs, monitor that the care packages continue to meet their needs and review the planned care to ensure it provides a high standard of quality and safety.
Main duties of the job
The post holder will be responsible for a team delivering assessments of individual's eligibility for Continuing Healthcare and Funded Nursing Care and for reviewing care packages for existing patients. The post holder will ensure that assessments and reviews are carried out in a timely manner according to national specifications.
The post holder will work with all stakeholders including patients and, where relevant, their families, to ensure that funding decisions are taken in a transparent and coherent manner.
The post holder will ensure that the assessor team keep accurate records and are able to demonstrate that funding recommendations are evidence based and made according to the criteria described within national CHC frameworks.

The post holder will contribute to the clinical management function of the CHC service, forming part of a senior duty roster to handle fast track applications, review checklist referrals, manage complex cases and sign off assessment ratifications.
The post holder will ensure that all care plans are in place which are up to dayte and support the clients current care needs. They will also ensure there is a regular review process in place for the care plans and where these have not been updated or are omitted from the clients file, they will ensure they are in place promptly. Further to this, the individual will ensure that where a need for on-going referral to another specialist is required (e.g. DOLs, Safeguarding, SALT, OT, CHIT, Community LD specialist Team), that this is completely promptly, with the outcome of any referrals being included in the CHC assessment as deemed appropriate.
