Job summary
The Transfer of Care Bureau (ToCB), based at the Princess Royal University Hospital (PRUH) brings together key specialist teams from across the health and social care sector under one umbrella to facilitate discharges of patients with ongoing complex health and care needs. These include a team of Discharge Coordinators, London Borough of Bromley Hospital Care Management and Brokerage Team, Acute Therapies, Business Intelligence and Administration Team, St Christopher's end of life in-reach, Discharge to Assess and voluntary and community sector representation.
The post holder will be responsible for the operational management of the ToCB including direct management of Care Management and Discharge Coordinator Team leaders and matrix management of all other professionals who are part of the infrastructure. The post holder will also be required to inform and lead the strategic development of out of hospital pathways and provision to ensure community services remain responsive to the demands being placed on the acute trust.
The post will be required to provide key leadership and operational management around implementing and taking forward the recommendations of the review to formalise the ToCB structure as well as implementing national policy drivers around improving hospital discharge and reducing Delayed Transfers of care.
Main duties of the job
The post holder will be responsible for:o Day to day running of the ToCB.o Managing escalations to facilitate discharge.o Managing systems and processes to ensure Delayed Transfers of Care (DToC's) are minimised, and that there is robust monitoring and reporting in place.o Provide leadership and management to the specialist teams within the ToCB umbrella including continuing health care, discharge co-ordinators, social care and therapies.o Strategic leadership for service development and service planning / delivery for each specialist team within the ToCB and how they integrate as a whole.o Develop resilience and escalation plans in order to be responsive during times of high demand and pressure.o Workforce planning and developmento Professional leadership and management of the acute therapies department.o Providing strategic leadership to the out of hospital network economy to ensure community services are responsive to need.
