Job summary

Live Well Greenwich is our whole system approach to prevention. This approach aims to unite efforts across organisations to maximise opportunities for prevention and to reduce health inequalities through existing strategies, infrastructures and services; bringing them together in a single comprehensive approach working at individual, community and population levels. The primary responsibility of the role is to provide strategic and operational support, and clinical input for in the interest of Greenwich residents with regards to the south east London (SEL) Live Well agenda.The Live Well Greenwich role will be responsible for providing the care/clinical system voice to a range of programmes that focus on prevention, population health and reducing health inequalities. It will also work alongside integrated commissioners to develop our jointly funded, integrated personalised care infrastructure (social prescribing/care coordination) that is delivered through the voluntary sector and built upon strength based approaches. The role also encompasses clinical leadership for medicines optimisation.The post-holder will work alongside colleagues from public health, primary, community and secondary care, commissioning, and the VCSE to develop the necessary relationships to support delivery of improvements in population outcomes through proactive care and prevention.

Main duties of the job

o Care/Clinical input into the development and commissioning of the Live Well Greenwich infrastructure and its links to a broader range of initiatives/support including online digital support, wider system links and the voluntary and community sector. This role will work closely with Primary Care colleagues who jointly fund the service. o Provide system leadership and influence as part of a partnership with other care and clinical professional leads.o Work on an agreed set of system prevention priorities, aligned to the Greenwich Health & Wellbeing Strategy, NHS Long Term Plan and other national and local policy directives, including the Core20 Plus five, Greenwich recovery plan and the vital five initiatives.o Ensure that the voice of residents and people with lived experience is heard and acted upon as part of the work.o To ensure that the Healthier Greenwich Partnership contributes to the prevention agenda o Providing clinical leadership to support medicines optimisation implementationo Providing clinical leadership and approval of new medicines and pathways into SEL

Qualification and Training

Essential

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.
  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation in area of prevention, health inequalities, public health and behavioral change
  • Champions the principles of equality, diversity and inclusion both within and outside of the organisation. Provide effective, inclusive and compassionate leadership ensuring all voices are heard and creating and nurturing a culture where staff feel safe to speak up.
  • An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities.
  • Certificate / qualification in programme management, service improvement or transformation
  • Cross organisational and/or professional boundary collaboration experience
  • Good knowledge or cross professional transformation, digital solutions and innovative redesign.
  • Previous experience of being part of peer networks and collaborative working
