Job summary

In Lambeth, we are very fortunate to have 41 high-quality GP Practices, with a dedicated workforce who consistently achieve good clinical outcomes and satisfaction ratings. We also have strong at-scale provision and leadership through our 9 PCNs, our GP Federation, and our local LMC, who regularly show their enthusiasm and energy to get on with improving services for their patients.

We are working with local people and partners across health social care and the voluntary sector to improve health and wellbeing for all of our communities. Our key priority is to develop closer integrated and collaborative ways of working with all of our partners to implement Fuller and Lambeth Together's Health and Care Plan.

The Lambeth Primary Care Team is a key reason for this, having forged excellent long-term relationships across the local primary care system, and supporting Practices, PCNs, and the Federation to achieve their goals and deliver high-quality care for patients.

Main duties of the job

We are seeking an exceptional candidate who has the drive, focus, ability, and experience to deliver results to lead on Primary and Community Care for Lambeth, as the borough's Associate Director of Primary and Community Care.

The borough is large and diverse, with a registered population of over 450,836 patients. It is a dynamic and varied primary care programme covering Primary Care Commissioning, PCN Development, Key Enablers, and playing an important role in developing strategy linked to the development of "Lambeth Together" - the borough's ambitious integration programme.

We are looking for someone who will be a major contributor to the development and delivery, at pace, of transformed primary care services. Always striving for excellence, you will have significant recent experience in primary care at a senior level, be adaptable and resilient when under pressure, bring a growth creative mindset and experience to deliver service and system transformation, and a proven ability to work collaboratively within and across organisations, and leading teams.

The post-holder will support the Director of Primary Care & Transformation in leading the Lambeth Primary Care Team by creating, leading, managing, and delivering on all aspects of the ICB's Primary Care programme and responsibilities in the borough.

Importantly, the ICS seeks to be connected to the communities we serve (circa 1.92m residents) and work with the widest possible range of community, voluntary and third sector groups and organisations in each borough.
