Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The Clinical/Care Professional Lead for planned care and diagnostics is a key post within the Lambeth Together Care Partnership, working across all partners including the NHS (including primary care), Local Authority, and our communities.

The postholder will work collaboratively with a number of existing clinical and care professional leaders including those from; the Acute Provider Collaboratives, Specialist Networks, colleagues and counterparts in neighbouring Southwark Place, SEL ICS colleagues and other stakeholders including region and national in supporting and providing clinical input into realising improvements around planned care pathways, services and access to diagnostics, whilst ensuring the views of colleagues and peers are heard and taken into consideration when delivering improvements and change.

Main duties of the job

o Work collaboratively with the Lambeth and Southwark Planned Care and Urgent Care Teamo Working across key teams within our Lambeth Together Care Partnership to help them ensure the delivery of our local and SEL objectives.o Key APC and specialist network meetings applicable to the current focuso SEL Planned Care Forumo Specialist programme / project meetings pertaining to current focuso Act as a key point of contact for planned care and diagnostics related to issues and opportunitieso Working actively with providers to address challenges in a collaborative and respectful way.o Working directly with practices and services who are struggling with an aspect of planned care and diagnostics delivery including communicating to practice that they are an outlier in respect to a specific performance indicator.
