As part of delivering the LD&A /Transforming Care Agenda, the NHS South East ICB are leading on the new all-age Dynamic Support Register (DSR) for people with learning disabilities and or autism who are at the highest risk of a hospital admission across Lambeth, and on innovative ways to support discharge from hospital.
You will have a key role working with children, young people and adults who have learning disabilities and/or autism to support discharge, prevent admission and to support networks of professionals around the individual to ensure they receive coordinated, joined up healthcare.
You will link with all relevant local partner organisations within the Integrated Care System ICS) including: Local Authorities, Health Trusts, Education and Police, ensuring the ongoing development of the DSR continues to be co-produced with experts by experience.
The DSR helps the system to provide increased vigilance and improved strategic planning for those at high risk of placement breakdown and/or hospital admission, ensuring proactive monitoring and support reducing the risk of admission.
Main duties of the job
The aim of the Learning Disability and Autism Programme is to improve the lives of children, young people and adults with learning disabilities and/or autism who display behaviours that challenge, including those with a mental health condition.
The programme has three key aims:
- To improve the quality of care for people with a learning disability and/or autism
- To improve the quality of life for people with a learning disability and/or autism
- To enhance community capacity, thereby reducing inappropriate hospital admissions and length of stay
