Job summary
Organisational context
Lewisham has one full time part time (18.75 hours per week) vacancy for a nurse to work in the Continuing Healthcare team to implement the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.
You must be a Registered General or Mental Health Nurse with community experience in Older People's Services, be competent and knowledgeable in all aspects of the NHS Framework for Continuing Healthcare Funding.
As a Continuing Healthcare Nurse you will need to be able to write clear recommendations / reports regarding a patient's eligibility for NHS Continuing Healthcare. You will need to have highly developed written skills. You will also need highly developed verbal communication skills as you will be required to explain complex and often contentious /sensitive information including the outcomes of NHS Continuing Healthcare assessments and future care needs to patients and/or their families, and have knowledge of the appeals process. You will conduct meetings with families and patients to review CHC and FNC eligibility. You will arrange and actively participate in CHC assessments with other members of the MDT and type up reports for panel and work in collaboration with the Local Authority. You will conduct reviews on a 3 monthly and yearly basis and write reports around findings.
You will need a current clean driving licence.

Main duties of the job
You will be responsible for those patients who meet the eligibility criteria for NHS Continuing Healthcare. This will include the assessment of care needs and the development, implementation, evaluation and reviews of programmes of care.
The post holder will work as part of the continuing care team to develop the role and to contribute to the development and implementation of a robust NHS continuing healthcare process and assessment for NHS funded care.
Continuing Healthcare patients will have complex and/or intense health care needs and/or unstable fluctuating conditions, and/or unpredictable, and/or progressively/rapidly deteriorating; aggressive disinhibited behaviour which is difficult to control; unstable symptom control or severe continuous symptoms or a very high level of medical, nursing or therapy intervention with monitoring from one or more health professionals and/or their health needs will require a high level of professional time.
As you may be aware NHS Continuing Healthcare is very contentious and therefore you will need to be assertive in order to deal with challenges from patient's relatives and other professionals.
