Please note that this is a hybrid role with an expectation of at least two-day per week in the office (Laurence House, SE6).
We are looking for an experienced senior nurse with experience of children's mental health and emotional well-being to join our Children and Young People's Joint Commissioning team, initially for a two-year fixed term basis. This is an exciting opportunity to work across NHS South East London and Lewisham Council to make a real difference to children's mental health and emotional well-being.
Reporting to the Head of CYP Joint Commissioning, the Clinical Nurse Officer will support the development of children and young people's mental health and emotional well-being pathways and our aim of ensuring children and young people receive the right support at the right time. They will provide expert advice and guidance to joint-funded care placements and ensure there is adequate and timely health input. There will also be opportunities to support and lead on the develop of new process and lead on the borough's CYP Dynamic Support Register (DSR).
The post holder will be employed by NHS South East London on behalf of Lewisham Council.

Main duties of the job
The post holder will ensure that the outcomes for children and young people and those with mental health needs are maximised by working to improve quality of life; school absences; emotional and physical health concerns; personal autonomy and involvement in further education and employment; and independent living and community inclusion.
The post holder will provide expert information, advice and guidance on matters relating to CYP mental health for the ICB, Local Authority, health care providers, families and other local agencies and organisations, as well providing support to the Shared Funding Panel (LBL/SEL ICB)
The post holder will be part of the Children's Joint Commissioning Team and will have a role when reviewing and quality assuring all aspects of delivery to support children and young people with enduring mental health concerns.

Shared Funding for Placements and Complex Care Packages
The Council and SEL ICB (Lewisham) have a Shared Funding Panel for complex cases with representation from joint commissioning, SEL ICB, social care, education, and finance.
The post holder will lead the SEL ICB (Lewisham) joint funding process to review health assessments and costs, provide clinically informed oversight to both the process and associated health budgets. The post holder will contribute to placement planning discussions to avoid crisis situations and support step down from hospital / inpatient services.
Responsibilities to include: -
- Coordinate and oversee shared funding placement / complex care packages on behalf of the SEL ICB (Lewisham).
- Oversight of £1m health budget to support complex care packages and placements.
- Attend and provide clinical advice for placement planning (CLA / SEND) professional meetings when theres a significant health component to prevent placement / family breakdown or support step down / prevention of crisis.
Dynamic Support Register (DSR)
The post holder will have clinical oversight and management of the children and young peoples Learning Disability and Autism Programme Dynamic Support Register (DSR) and Transition to Adulthood.
Responsibilities to include: -
- Oversight of the Dynamic Support Register, by working with the allocated business support officer, the post holder will ensure that the DSR is up to date and contains the correct level of information for individual CYP
- Chair the six weekly DSR meeting to ensure effective co-ordination across the professional network
- Coordinate and chair Lewisham CYP Community Education Treatment Reviews (CETRs) for outpatient cases and will attend and represent commissioning (SEL ICB) all Lewisham inpatient CETRS, which are chaired by NHSE.
- Attend the monthly in-patient surgery to provide and receive updates on those young people on the DSR who are in hospital.
- Working closely with the Designated Clinical Officer (DCO) and the LDA Programme regional operations group, keeping abreast of the wider programme delivery and implications for place.
Transition to Adulthood Pathways
The Council and SEL ICB (Lewisham) are commitment to supporting young people with special educational needs and/or disabilities aged between 14 and 25 years to prepare for adulthood. The post holder will have oversight of key cohorts within Childrens Services, notably Children Leaving Care, Young People transitioning from Childrens to Adults with Mental Health Services and Young People transitioning from youth offending services.
Responsibilities to include: -
- Oversight and involvement in CYP cases on adult DSR between ages of 16 21 to support effective transition.
- Attend placement planning (children looked after) professional meetings when theres a significant mental health component to ensure smooth transition. In addition, provide advice and information on suitability of placements to meet individual CYPs needs.
Children and Young Peoples Continuing Care
The postholder will provide clinical input for CYP Continuing Care (CC) assessments and help identify the appropriate support necessary to meet their needs.
Responsibilities to include: -
- Reviewing the pre-assessment to help identify cases to be escalated to full CYPCC assessment.
- Providing clinical perspective in panel decisions on eligibility for continuing care.
