This is an exciting opportunity for a highly motivated, dynamic and passionate Practitioner to join the All age complex and continuing healthcare (AACCC) team. The work stream is predominantly ABI and complex cases. Pre work for the NHS CHC process, that includes rehabilitation reviews and promoting patient to attain their potential base line.

ABI and Complex case Practitioners are an integral part of the AACCC Complex care team working alongside other members of the Team to support the Complex Care Lead, in achieving best practice and enable BOB ICB to fulfil its legal requirements.

Main duties of the job

This position requires the post holder to have a knowledge of working with patient requiring rehabilitation, particularly around acquired brain injury, spinal injury and complex cases. The role holder will need to understand principles of MCA and be willing to support team members with the responsibilities of a BIA if required. The applicant will have substantial experience of conducting patient assessments.

Main duties of the job:

Support the team leads with reviewing and assessing those patients engaged in ABI rehabilitation, and ensuring that the goals are monitored and reviewed, ensuring optimum patient journey to achieve potential baseline.

Ensure that care packages for complex cases are reviewed and right sized to ensure optimum potential is gained assuring baseline is achieved.

Support the complex case team and the wider AACCC team and ICB to maximise care outcomes for patients, driving up quality, and reducing costs with cost improvement programmes.

Assist the ICB to achieve value for money with right size care packages

Promoting patient to maximum independence within the remit of this role.

The role will involve travelling to meetings and patient homes as needed.
