Buckinghamshire, Oxfordshire and Berkshire West (BOB) ICB are looking to recruit 2 roles within
the All Age Continuing Care (AACC) Service, the post holders will play a crucial role in supporting the delivery of PHB's for individuals eligible under Continuing Care.
Main duties of the job
The roles will include:
Working
in partnership with and through clinicians at Place to implement Personal
Health Budgets, including supporting and advising on the development of care
and support plans to meet identified health and care needs/health and wellbeing
outcomes.
Supporting
and educating case managers on the calculation of indicative costs, providing
recommendations and financial analysis to All Age Continuing Care team.
Supporting
to monitor and review client financial information to ensure spend is in line
with assessed need and alerting clinical teams to any changes identified.
Providing
advice and guidance to clients and clinicians as and when required, acting as
an advocate to ensure the best possible outcomes can be achieved.
The role will involve travelling to meetings and patient homes as needed.
The PHB Officer will work closely with internal and external stakeholders,
contributing to service improvements through regular supervisions and ensuing
that services are delivered in alignment with local and national guidelines.
