Personal Health Budget (PHB) Lead (2 WTE)

37.5 hours per week

Job Type Secondment or Fixed Term Contract to end June 2024

Buckinghamshire Oxfordshire and Berkshire West (BOB) ICB are looking to recruit 2 roles within the All Age Continuing Care (AACC) Service. The post holders will work across the BOB footprint to ensure that appropriate DOLs applications and made and progressed for individuals who lack capacity and are eligible for AACC.

Main duties of the job

The PHB Lead will need to work closely with the current Continuing Healthcare team to ensure that personal health budgets continue to be embedded as part of the normal Continuing Healthcare process.

Provide specialist support to Continuing Healthcare teams in Oxfordshire and Berkshire West in the case management of their clients who have or wish to have a personal health budget; through coaching and mentoring, providing specialist information and guidance as required.

To work in partnership with Oxfordshire and Berkshire West Continuing Healthcare management teams to continually review operational practice and process for personal health budgets, to ensure:

  • They are delivered equitably, efficiently, and effectively.
  • they enable people to have choice and control over decisions affecting their health and wellbeing and deliver personalised outcomes.
  • Support external agencies, organisations and people receiving NHS Continuing Healthcare in all aspects of the process to setup and manage a personal health budget.

  • Provide specialist support and advice to other healthcare teams and their partner organisations in the development, implementation, and delivery of PHBs for people outside of NHS continuing care.

  • Represent BOB ICB management team in the development, implementation, and evaluation of operational procedures relevant to the other areas of service delivery within the Continuing Healthcare service (e.g., shared care, transition planning)
