Job summary

The NHS Berkshire West Continuing Healthcare Service, part of NHS Buckingham, Oxfordshire and Berks West Integrated Care Board (BOB ICB), is recruiting a Team Leader based at their Reading office, although work is a mix of home/office working. This post will report to the Assistant Director, Continuing Healthcare.

The Continuing Healthcare Service (based in Reading) covers Wokingham, Reading, Newbury and surrounding rural areas. The Berkshire West Continuing Healthcare Service is a relatively small, friendly team working in a specialized NHS area. This Service is part of the wider BOB ICB where there is currently a transformation process underway to ensure similar working practices across the ICB so there may be wider opportunities within ICB.

Main duties of the job

The post holder will be responsible for supporting the Assistant Director, Continuing Healthcare in the day to day operational management of the Continuing Healthcare Service for Adults and Children. This important role involves working with the most vulnerable individuals in society and the NHS.

A key aspect of this post will be to provide day to day leadership to the Continuing Healthcare Service for Adults and Children and to provide skilled and experienced support to the Continuing Healthcare and FNC Assessors, as well as to the Assistant Director, Continuing Healthcare, the Continuing Healthcare Panels for adults and children, and others as appropriate.

The Continuing Healthcare Service works with its Local Authority partners and other stakeholders to implement the National Framework for NHS Continuing Healthcare and has a continual programme of training to Health and Social Care partners. To continue and follow on from this work, the postholder will need to develop and maintain good working relationships internally and externally to the ICB.
