post holder will be responsible for:
Undertaking NHS Continuing Healthcare/Funded Nursing Care
assessments,
care planning and the evaluation of packages for patients
eligible
for NHS Continuing Healthcare.
Support the investigation of complaints and review of NHS CHC
retrospective
cases, appeals and independent panel review process.
Support the delivery of personalised commissioning to include transforming

arrangements), Personal Health Budgets, the coordination of
