You will play a leading role in enabling the highly respected Leeds Health & Care Partnership to take the next steps in population health and transformation. Working with senior executives across the partnership you will bring a deep understanding of value-based healthcare and population health management with the skills to translate this into at scale change across a city of 850,000 people.
Working collaboratively with colleagues in Leeds and across West Yorkshire you will play a pivotal role in identifying population risks and opportunities to inform our collective transformational priorities for various populations. You will support the development of neighbourhood health and ensure the deeper integration of services through effective and innovative resource allocation and incentives thus improving health outcomes for different populations.
You will lead a team that works closely with the medical directors and clinical leads to ensure that the partners progress the implementation and benefits realisation of local plans acting as a catalyst for transformation.
The Leeds Health & Care Partnership is a strong partnership bringing together the ICB with Leeds City Council, Leeds Teaching Hospitals Trust, Leeds Community Health Trust, Leeds and York Partnership Trust, the GP Confederation, Leeds Universities and Third Sector.
Main duties of the job
Reporting to the Leeds Place accountable Officer you will be a member of a director team in Leeds with delegated accountability of a c£1.7bn budget. Within that team your specific accountabilities will be:
The co-creation and consistent application of the vision for a strategic population health management approach across the city to facilitate delivery of improved outcomes.
Leading the city in prioritising, scoping, and enabling delivery of a constantly refreshed set of major programmes that address key population and system risks.
Ensuring benefits are clearly articulated, owned, and tracked for all major programmes, and managing the organisational, partnership and system conditions required for benefits to be released.
Leading the establishment and effective implementation of contractual, financial, and risk-sharing mechanisms that align provider incentives to both person and system needs, without creating unwarranted risk to partners.
Establishing an understanding of the Leeds health and care market, supporting interventions in line with our strategy that deliberately build or protect capability in specific sectors or providers and the overall system.
Ensuring that plans and reporting connect across organisational, place and system priorities, providing assurance that balances short term performance with long-term impact.
Delivering financial sustainability across key multi-million-pound budgets including primary care.
