We have a fantastic opportunity for a highly motivated dynamic Section 117 aftercare case manager , to join our clinical/Nursing team and bring forward their expert leadership and management experience. Applications will be welcomed who already have experience in management and leadership role in Section 117 after care. The post holder is responsible for overseeing the clinical governance of the cohort of individuals that receive packages of care funded under sec 117 after care. The primary duty of the post holder will be to manage the clinical governance of sec 117 case load. This will include managing all referrals for sec 117 aftercare that are submitted to the ICB. Ensuring the care and support plan from providers are in line with the persons assessed sec117 aftercare needs.

Main duties of the job

The successful candidate will be responsible for providing management, clinical governance and overseeing Wakefield places section 117 after care caseload. Working in partnership with a local authority ensuring Wakefields place statutory responsibilities under section 117 after care are met.

This remit includes the development, implementation, evaluation and reviews of s117 programmes of care. This will additionally include the administration of processes to designate and allocate and to advise on/managing patients.

The post holder will work as part of the clinical team to develop the role and to contribute to the development and implementation of robust Section 117 aftercare processes . The post holder will additionally support the development of integrated commissioning arrangements with the Wakefield Local Authority, including the development of Community Based Commissioning arrangements and associated strategies.

The post holder will assume the responsibility for providing clinical support and leaders

Qualifications

Essential

  • Educated to post-registration/graduate level or equivalent in experience
  • Hold a RNLD or RMN Registered Nurse Qualification
  • Current and relevant NMC registration

Desirable

  • Post Graduate qualification or specialist training
  • Recognised teaching/mentoring qualification or equivalent experience

Skills & behaviours

Essential

  • Able to analyse and interpret complex business data
  • Able to deal with difficult and contentious issues in a professional manner ensuring the correct courses of action are undertaken. Skills for managing projects, ensuring they remain on track and withi

Experience
