Job summary

The All Age Continuing Health Care Service at WYICB Wakefield Place is primarily responsible for assessing and commissioning AACC and personal health care services for our population. The service works in close liaison with our local authority partners, Care Homes and Domiciliary Care Providers to support a range of care placements and integrated care services. If you are looking for new challenges, passionate about patient care and achieving the best health outcomes for patients, an opportunity has arisen for a dynamic, motivated, experienced Registered Nurse to lead the NHS All Age Continuing Healthcare clinical commissioning function. As Clinical lead, you will join our clinical/Nursing team and bring forward their expert leadership and management experience. Applications will be welcomed who already have CHC leadership and management experience or who are able to demonstrate their aptitude to lead and influence a busy nursing team. The successful candidate will be responsible for providing management and leadership and overseeing a team of professionally qualified nurses in the delivery of NHS All Age Continuing Healthcare and for Deputising for the CHC Head of Service as required.

Main duties of the job

The post holder will work as part of the clinical team to develop the role and to contribute to the development and implementation of robust NHS healthcare processes both at Place and across the ICB. The post holder will work as part of the NHS Continuing Healthcare (CHC) Team and will be responsible for providing clinical leadership, expertise, advice, supervision and line management of the CHC Nurse Team Leaders. The post holder will report to the Head of AACC and provide the clinical oversight and management of our nursing team, providing support and professional clinical leadership to the WY ICB Wakefield Place CHC Team. They will be responsible for driving any clinical innovations and changes to improve our service, the correct interpretation of the National Frameworks for AACC, achieving our quality standards; providing high quality services to the population of Wakefield ensuring that our eligible patients receive the right care, at the right time in the right environment. They will be responsible for liaising with patient and their families, appeals processes and complaint investigations.

About us

The post holder will assume the responsibility for providing clinical support and professional nurse leadership as required for a designated work streams and care groups, so as to support the commissioning priorities/principles of Wakefield Place, the wider WY ICB and the work programs of the team. The role will link closely with the CHC senior management team working across systems to ensure seamless patient pathways are achieved. You must be an NMC Registered Nurse and have extensive experience in multi-disciplinary/multi-agency working and NHS CHC. The successful post holder will have the support of an experienced senior clinical and non-clinical commissioning team.
