Job summary

The role of the Medicines Optimisation Dietitian is to offer support to the Medicines Optimisation Team across Bradford District and Craven, and strategic support to the wider West Yorkshire ICB to ensure that our primary care colleagues have access to high quality advice and guidance in order that they may offer good clinical care to patients requiring nutritional support. In conjunction with all team members the post holder will build strong relationships with stakeholders, local dietetic services, GP practices and Primary Care Networks within our Health and Care Partnerships to promote high quality, safe and cost effective prescribing of nutritional products informed by evidence based practice and national guidance. By facilitating the development and implementation of the of local guidelines, this will ensure that patients get the right choice of product at the right time thus improving patient outcomes within financial constraints.

Main duties of the job

The post holder will work in conjunction with other health care professionals across our health and care system to develop, and promote implementation of guidelines, information and services that support patients to receive high quality dietetic care. This may involve liaising with local services, other colleagues across and outside of our ICB, manufacturers and other stakeholders, interpreting prescribing data, developing guidelines and ensuring that our practices and PCNs are assisted to understand and implement recommendations.

In order to undertake the role, the Medicines Optimisation Dietitian will be able to demonstrate skills and specialist knowledge acquired through a BSc degree in Nutrition & Dietetics and be registered with the Health and Care Professions Council (HCPC). A post-graduate Diploma in Dietetics and being a member of the British Dietetic Association and primary care experience are desirable. They will be accountable for their own professional actions, train and develop colleagues, work independently as well as within teams and through the support of pharmacy technicians and pharmacists, will organise their own workload and that of their colleagues in the Medicines Optimisation Team. Excellent written and verbal communication, keyboard, analytical and interpersonal skills are essential.

The role is 0.4 wte (15 hours per week)

Support the development of high quality, safe, appropriate and cost-effective use of nutritional products in collaboration with all other stakeholders in the local health economy.

Lead on the implementation of appropriate prescribing of nutrition products across Bradford District and Craven including developing guidance & policies based on national guidance across place and the wider ICB

Work closely with health professionals in secondary care on interface nutritional issues

Liaise and update Optimise Rx with regard to dietetic products

Provide medicines optimisation dietetic support to the @assist project.

Work as part of a multidisciplinary team within the ICB providing guidance on any dietetic related issues

Assist in identifying issues and concerns in dietetic prescribing activity and variance from policy in primary or secondary care.

Have the opportunity to be involved in, and in some cases lead on, research, audits and other service development initiatives both within and out-with the dietetic department

You will be required to undertake teaching/training of other health professionals, including primary care prescribers.

Ability to work across a West Yorkshire ICB footprint.
