The post holder will:

Assist the ICB in delivering its commissioning responsibilities as required in The National Framework for Continuing Healthcare and Funded Nursing Care.

Undertake clinical assessments and reviews of Continuing Healthcare eligibility for adults of all ages utilising the National Framework for NHS Continuing Healthcare and Funded Nursing care.

Work with professionals, providers, individuals and their families and/or representatives to make determinations regarding nursing care need requirements for new and existing service users.

Support in monitoring the standards of care delivered by care providers, giving advice and guidance to care staff regarding individual service users care needs in line with clinical governance.

Operational

Has an understanding of other agencies policies and procedures and to work in accordance with relevant legislation and acts, including The Community Care Act and Mental Health Act.

To have knowledge of record keeping systems for audit and resource development at local level

Main duties of the job

Clinical Utilise experiential knowledge and clinical skills to ensure effective application and interpretation of the National Framework for the NHS Continuing Health Care and NHS Funded Nursing Care.

Work collaboratively with other health and social care colleagues across the system to assist in the determination of an individuals care needs in accordance with the National Framework for Continuing Healthcare and Funded Nursing Care.

Educates, informs, and advises patients/carers/relatives to support informed choice.

To ensure that care packages are monitored on behalf of the ICB and that the service that has been commissioned for that individual is being delivered as agreed.

To identify any concerns about the care of individuals and take appropriate action to advise on improvements required and where necessary inform the relevant agencies in accordance with local safeguarding agreements.

Maintain accurate and contemporaneous records and facilitate, support, maintain and improve record keeping systems.

Communication

Assess for and communicate sensitive and complex medical/nursing information/needs effectively to patients and carers. Even where there may be barriers to understanding such as mental health problems.

The post holder will require good verbal and written communication skills to communicate with patients, their families and other professionals. The production of accurate, detailed and comprehensive reports
