Job summary

This new and exciting post, funded by MacMillan for 22 months is to support the system palliative and end of life care strategy to deliver improved experiences of care and support by working with our excellent voluntary, community and social enterprises (VCSE) in Cornwall and the Isles of Scilly .

Working with the integrated care system (ICS) end of life manager and the wider system strategic alliance, this post will lead on a range of workstreams with the VCSE including system transformation, quality improvements, new models of care/ support at place and redesign programmes from concept to procurement.

Main duties of the job

Support the system palliative and end of life care (PEoLC) strategy

Lead on a range of new workstreams including new models of care, service development and redesign programmes from concept to procurement in order to improve experiences of care for our people, families and carers.

To support the commissioning team to deliver sufficient capacity to meet demand whilst addressing variation.

To work closely across the ICS and region with other clinical lead(s), service providers, patients, families and carers to understand current provision of end of life services and to implement new models of care, appropriate to the needs of the local population.

  • Educated to master's level or equivalent level of experience.
