Job summary
We are looking for a positive and enthusiastic professional with a nursing, social work or an allied health professional qualification who is looking for a new challenge or next step in their career to join the Continuing healthcare (CHC) assessment team as the team manager. This role is currently offered as a secondment from our system partners.
Using your leadership and motivational skills the postholder will manage a busy team of CHC coordinators and clinical support staff. You will provide day to day line management, undertake quality assurance through auditing and supervision.
You will have extensive experience of holding a case load and of managing complex needs within health and social care. You will have experience of managing a budget in relation to designing and arranging safe and effective packages of care. You will promote Personal Health Budgets and personalisation, empowering people with complex health needs to live their best lives.
Our ideal candidate will be able to work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). The candidate will have experience of managing a team.
Main duties of the job
This important role is central to the delivery of CHC across Cornwall and Isles of Scilly with a primary objective of ensuring fair access to CHC with better outcomes and better experiences for our patients and ensuring that our statutory responsibilities are met.
You will promote good professional practice in CHC by advising and liaising with other health providers, care providers, individuals and their representatives and Adult Social care colleagues. You will be responsible for managing and coordinating assessments and commissioning care packages.
To be successful you will be able to demonstrate the ability to remain calm, to handle emotive, complicated, and sensitive issues. You will need to show tact and diplomacy, be able to manage a complex and varied workload and be able to negotiate with confidence.
As part of the county wide team you will need to work flexibly to respond to the demands of the service to support the continuing healthcare process. The ability to drive to various locations across Cornwall is essential.
