Job summary
This is a part-time position for a consultant with a special interest in lipids management. The consultant will be driving the development and the implementation of an integrated lipid service across HWE ICS, working closely with a team of clinicians and non-clinicians. The job owner will have to engage with providers in primary and secondary care to support the implementation of NICE guidance on lipid management across the system and contribute to the setting up of an integrated service that will improve cardiovascular disease prevention in HWE ICS.
The Lead Consultant will provide essential clinical guidance and expertise into the development of service specifications for the ILS, clinical pathways, clinicians education and work with colleagues to identify the best use of currently available resources to improve performance across certain areas of CVD Prevent.
Main duties of the job
Contribute to the development of a training and education program for primary care.
Deliver primary care training sessions via primary care training hub, PCN training afternoons, locality-based meetings.
Record training webinars for dissemination to colleagues in primary care
Offer clinical guidance for the development of the ILS across the ICS.Lead discussions with clinical colleagues to agree the clinical pathways underpinning the service delivery model.
Contribute to the development of referral criteria into the ILS.
Analyse the currently available date supporting the development of the business case.
Promote the implementation of the lipid management guidelines across the ICS. Attend stakeholders meeting to present the ILS and encourage engagement.
Attend system wide and reginal meetings to present the ILS project and update on implementation.
Support communication to primary care and other stake holders regarding the development and implementation of the ILS.
Offer guidance on the practical implementation of the service across providers in the system.
