Job summary
As a Funded Nursing Care Assessor you will play a key role assessing
and reviewing people's eligibility for NHS Funded nursing care in line with The National Framework for NHS Continuing
Healthcare and Funded Nursing Care (FNC) and where appropriate, referring onwards
for a Continuing Healthcare assessment.
Main duties of the job
A Funded Nursing Care assessor will
play a key role in supporting the Personal Health Commissioning (PHC) service and
delivering an effective and competent level of service and consistently
delivering a client-focused service.
You will work within an established
supportive team, completing assessments face to face or virtually.
You will be assigned
approximately four nursing homes and will have the opportunity to liaise and work
closely with nursing home staff to ensure initial assessments and reviews are
undertaken in a timely fashion and ensuring a good standard of care is being
delivered. There will be liaison with patients, families, providers and
potentially other professionals within the health and social care sectors.

- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area, ideally Continuing Healthcare
- Nursing qualification or Allied Health Professional e.g. Occupational Health, Physiotherapy etc. or registered Social Worker
- Knowledge of NHS Continuing Health Care and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care
- Evidence of Continued Professional Development
Experience

- Extensive knowledge of specialist areas acquired through experience and training plus further specialist knowledge of the Personal Health Commissioning process
- Knowledge, understanding and ability to implement the Personal Health Budget process
- Experience and understanding of evaluating and measuring the performance of health services
- Experience of working effectively with clinicians, Health and Social Care colleagues and other partner agencies
