Job summary

The CCGs in Bedford, Luton and Milton Keynes have come together under one governing body and senior team referred to as BLMK CCG. We are an exciting new organisation putting the needs of communities at the heart of our commissioning arrangements, by working with local partnersacross health social care and the third sector.The Individualised Care and System Flow department includes a wide range of services encompassing unplanned care, (i.e. emergency care), discharge and intermediate provision, acute services and pathways for those living with frailty including those for older people and/or those at end of life and personalised care through the commissioning of care packages and placements via personal health budgets or continuing healthcare funding.The post holder will play a key role in supporting the CHC department's operations by managing and developing the Continuing Healthcare clinical team, working in conjunction with external stakeholders, to implement an effective service, ensuring the delivery of key quality outcomes and strategic objectives.The post holder will line manage a team of CHC assessors ensuring delivery of patient centred assessments in order to identify any primary health care need which may result in eligibility for CHC ensuring the national framework for NHS Continuing Healthcare and NHS-funded Nursing Care is implemented and complied with according to theprinciples and processes.

Main duties of the job

Key Duties and ResponsibilitiesLeading and managing a team of Continuing Healthcare assessors ensuring a high quality service is delivered, supporting and developing staff to achieve continuous improvement.Maintaining constructive relationships with a broad range of internal and external stakeholders including Health and Social care services.Contributing to performance improvement, taking a lead for identified areas where agreed, and overall day to day management of the Continuing Healthcare team performance.

About us

As a member of the Continuing Health Care team the role will include working across our four Local Authority areas as well as potential visits to clients that live out side of the region.

The CHC team operates a hybrid working model that supports flexibility in its approach to sustaining a workforce to meet the service needs, this includes home and office working as agreed with the CHC management team.

Contributing proactively to the management of resources by ensuring that all Continuing Healthcare decision making is properly recorded and maintained.Analysing, interpreting and communicating national directives into processes, policies and presentations for application.Planning and deploying resources to manage individual and team performance to meet the demands of the service .Overseeing the management resolution of complaints, appeals and disputes, ensuring operational staff complete to a high standard.Managing a defined caseload.
