Job summary

The primary programme of work for this role will be the Shared Care Record Programme which is called Somerset Integrated Digital Electronic Record (SIDeR+) and connects all the significant electronic health records across Somerset. There will be other data sharing platforms which you will also be expected to support.

This role will be will be working across Somerset based at place and will be required to engage with and spend time embedded in the wider health and care community.

A key aspect of this role will be to implement transformational change by encouraging, training, and sharing and process mapping with health and care colleagues to embed use of SIDeR+ initiatives into clinical workflow and policies. An example of this will be helping to embed the electronic Somerset Treatment Escalation Plan (eSTEP) across the system.

You will be expected to be the liaison between NHS Somerset Digital team and health and care provider organisations. The post holder will be a strong advocate for transformational change through innovation and quality improvement and will actively promote and champion the development of a health and care data and digital information culture, as enablers of change and quality improvement across the Somerset system

This role will require a strong element of initiative, autonomy and confidence to help convene, problem solve, influence and lead colleagues from across different sectors and levels.

Main duties of the job

  • Help to embed SIDeR+ and other data sharing platform usage into each setting as part of clinical and care workflow
  • Work with people and act as a leader and catalyst for new ideas and use of data sharing platforms, this is not a technical role
  • Work closely with the SIDeR+ Programme Team supporting the Somerset Shared Care Record and other data sharing platforms including Digital Clinical Safety to lead safe deployment of digital services
  • Work with technical colleagues to de-obfuscate users and analyse SIDeR+ usage data and identify areas of high and low use in order to respectively collate viable current use cases and benefits for onwards promotion and best practice, and to increase service uptake and adoption
  • Listen to feedback from a variety of colleagues all at once, be able to distil this feedback for verification with the audience on the fly, and then commit in writing for onwards consideration as part of the service development workstream in every tier of the programme governance
  • Empathise with the health and care audience, understand the service pressures they are under, recognise their key tasks and responsibilities and tailor the active SIDeR+ service and other data sharing platform highlights as well as the future development roadmaps to their needs

The post holder will be expected to engage, listen, promote new ideas and facilitate service development and change across multiple teams and organisations All local health and care staff across acute, community, mental health, social care, hospice, ambulance, VCSFE and primary care settings will be in scope to connect with, helping to improve digital access (connectivity, confidence and literacy) and encourage information sharing to improve direct care as critical success factors

The postholder will need the confidence and capability to manage challenge from health and care professionals on the merits of the SIDeR+ shared care record and other data sharing platforms, and to be prepared to counter the challenges with proven benefits, clear understanding of service and platform content, but also be receptive to positive criticism in order to inform the future development of services to deliver the best experience and outcomes for our population

The overriding aim is to harness your experience, knowledge and skills, to help determine the most effective use of data, digital services and systems as part of local health and care pathways, to encourage local ownership of continuous quality improvement methods and support safe provision of the highest quality person centred care.

The ability to function in a complex health and care system will be essential You will be expected to be the liaison between NHS Somerset Digital team and health and care provider organisations. Work closely with the Digital Clinical Safety Lead to ensure safe deployment of digital services Work collaboratively with Acute, Community, Mental Health, Primary Care, Social Care, Hospice, VSCE and Communication colleagues on improvement initiatives to improve health and care pathways through use of data, digital tools and services, providing operational advice, engagement, mentoring and support

Compile and present activity data in a variety of graphical, pictorial and written formats that tell the story in an effective way to a number of different audiences, including clinicians, nurses, allied health and care professionals and residents of Somerset Interpret run charts, demand bell curves and other service performance data to profile health and care services and determine how SIDeR+ and other data sharing platforms can best support peaks and troughs over a variety of daily shift patterns and seasonal variations, and determine how to make a difference to each setting

Help to embed SIDeR+ and other data sharing platform usage into the fabric of each setting and set an active review state and contact so that each service continues to feel listened to, understood and receives updates

Key additional responsibilities of the role

Willingness and the ability to travel around health and care sites as well as work in HQ office and virtual environments

Ability to audit SIDeR+ and other data sharing platform data to ensure services are being accessed appropriately by users who can legitimise their direct care relationship to the person

Communication and engagement with health and care stakeholder staff and the public to optimise use and awareness of data, SIDeR+ and digital transformation opportunities, tools and services

Work with / initiate work packages to engage with frontline staff to adopt digital transformation using SIDeR+ programme and other data sharing platform initiatives, including management of health and care backfill requests if required

Foster digital inclusion by raising awareness of and promoting key digital programmes, projects, tools, operational goals and initiatives amongst health and care staff and the public Translate NHS England shared care record criteria and standards into requirements that direct the development roadmap of our countywide shared care record in order to maintain relevance and conformance

Examples include access for ambulance services to our shared care record via a national registry system, access to 111, Out Of Hours and Care Home records for people and assessing data sets recorded and display in comparison to what is expected by the Professional Records Standards Body (PRSB) Act as health and care champion and role model for using data and digital technology as enablers for safe and effective care, whilst remaining up to date with leading edge health and care data and digital developments

Manage clinical engagement and backfill budget, coding and tracking invoices from GP practices and other care settings, engagement events and other services Be a coach / mentor to enhance the performance and capability of others, leading to the development of learning networks to facilitate digital enablement and empowerment in all health and care settings.
