NHS Somerset ICB is seeking NMC Registered Nurses in the fields of Adult, Learning Disability and Mental Health to become permanent Band 6 Continuing Healthcare (CHC) Assessors to join the busy CHC team, which is part of the wider Quality and Nursing directorate.

This role will require you to undertake the breadth of CHC assessments, reviews and make eligibility decisions on patients (including regular face to face assessments across Somerset) for individuals applying for funding or who may already be in receipt of funding this will include those in receipt of a Personal Health Budget (PHB).

You will need excellent assessment and interpersonal skills, and be passionate about supporting individuals with long term complex needs, experience in CHC is desirable but not essential,

You will also have excellent organisational skills and a sound, knowledge of MS office packages such as Word, Excel, Outlook, Excel etc., alongside excellent written and verbal communication skills both virtually and in person.

Main duties of the job

  • To case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required, this will include those in receipt of a Personal Health Budget.
  • To undertake Continuing Healthcare (CHC) and Childrens Continuing Care (CCC) processes and assessments on patients who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track funding at end of life and those in receipt of a Personal Health Budget.
  • To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract ensuring value for money in all matters relating to packages for continuing NHS healthcare.
  • To have awareness of safeguarding and quality matters including the Mental Capacity Act (2005) and Deprivation of Liberty Safeguards (DoLs) (and Liberty Protection Safeguards) and participate in Best Interest Meetings, Safeguarding enquiries, Child In Need meetings, case conferences or other relevant meetings as necessary

The post holder will communicate and develop strong links with clinical services staff across the trust, as well as colleagues from external organisation such as Social Services, Mental Health Trusts, other ICBs and acute trusts, independent contractors and private care homes and providers. There will also provide contact with patients, carers and families.

  • To provide and receive highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism.
  • To case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required, this will include those in receipt of a Personal Health Budget.
  • To review and reassess individuals eligibility for NHS CHC, FNC and any other NHS funded placements
  • To undertake required assessments in support of the Deprivation of Liberty Safeguards (DoLS) , this may form a key part of the role.
  • To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract
  • To complete retrospective reviews as required and directed by team manager
