Clinical Reviewer and Complex Case Manager

Job summary

An opportunity is available for a Registered Nurse or Allied Health Professional experienced in supporting individuals with a learning disability and/or Autism to join the Clinical Review team within BNSSG ICB.

This post is a 12-month maternity leave cover and is 34 hrs per week over 5 days.

Our team works closely with system partners and care providers to deliver the NHS Care (Education) & Treatment Review policy and the Transforming Care Programme. The focus of this role is to help reduce the number of people with a learning disability and/or Autism in mental health inpatient settings and to support initiatives that prevent avoidable admission

We deliver some of our services and support by use of technology, so the post holder will be required to be IT literate and able to navigate remote platforms.

Our team is based in the heart of Bristol. The ICB supports hybrid working however, in order to deliver the functions of the services, flexibility and agility are required by the post holder to deliver all aspects of this role. Therefore, the post holder must be able to commit to being available to meet in person at short notice. This may include a variety of locations such as being in the office, in the BNSSG locality or out of area to support the delivery of the requirements of the role and ICB.

Main duties of the job

Communicate effectively and work collaboratively with a range of clinical and non-clinical professionals across multiple organisations.

Hold an all-age caseload, including individuals detained under the Mental Health Act and those in the community at increased risk of detention.

Use strong critical-thinking and decision-making skills to balance clinical need, identify risk and ensure effective care provision within available resources, liaising across the health and social care system to ensure a person-centred approach.

Support Multi-disciplinary discussions around individuals with heightened needs, ensuring a system knowledge of the dynamic support register and CETR/CTR processes; working collaboratively to avoid admissions for individuals presenting with Learning Disabilities and / or Autism.

A sound working knowledge of the Mental Health Act and Mental Capacity Act is essential.

Participate in regular reviews of the Dynamic Support Register with system partners to ensure accurate identification of individuals at risk of admission.

Apply and chair the Local Emergency Area Protocol (LEAP) for individuals at imminent risk of inpatient admission, supporting community services throughout the process.

Work collaboratively with local authorities and system partners to enable timely, personcentred discharges.

Complete quality assurance visits for all individuals in BNSSGs LD/A cohort, following Commissioner Oversight guidance.

Support achievement of NHSE discharge targets for the BNSSG area.

Maintain positive working relationships with a wide range of internal and external stakeholders.

Participate in internal and external working groups, providing analytical input and specialist advice.

Present complex information clearly to varied stakeholder groups.

Lead agreed areas of performance improvement and contribute to strategic planning, ensuring inter-dependencies and resource needs are identified.

Support the development of policies and procedures impacting the wider organisation.

Provide support to the wider Funded Care Team as required.

  • Educated to degree level in relevant professional qualification and a Health or social care professional registered with a nationally recognised regulatory body (e.g. NMC, HCPC, Social Work England).
  • Evidence of continued professional development.

  • Experience of managing risks and reporting.
  • Experience of drafting briefing papers and correspondence to Senior Management team.
  • Understanding of the public sector.
