Job summary

An opportunity has arisen for a registered nurse or allied health professional with experience of supporting individuals with a learning disability and/or Autism to join the Clinical Review team within BNSSG ICB.

Our small but effective team work with our system partners and care providers to support the functions of the NHS Care (Education) & Treatment review policy and the Transforming care programme. Our focus is to reduce the number of people with a learning disability and/or Autism who are in mental health inpatient settings and to support workstreams associated with admission avoidance to prevent unnecessary or inappropriate admissions...

We deliver some of our services and support by use of technology, so the post holder will be required to be IT literate and able to navigate remote platforms.

Our team is based in the heart of Bristol. The ICB supports hybrid working however, in order to deliver the functions of the services, flexibility and agility are required by the post holder to deliver all aspects of this role. Therefore, the post holder must be able to commit to being available to meet in person at short notice. This may include a variety of locations such as being in the office, in the BNSSG locality or out of area to support the delivery of the requirements of the role and ICB.

Main duties of the job

The role requires strong communication and administrative skills and experience of working with a number of agencies and organisations. The role will involve liaising with clinical and non-clinical staff involved in an individual's care and the skill to make links and facilitate commination between different areas and organisations.

As a member of the team, you will hold a caseload of individuals - all age, which will be a combination of those who are detained under the mental health act and those within the community whose risk may be escalating or may be at risk of detention.

This post will challenge your critical thinking and decision-making skills, requiring you to weigh up clinical need against resources and time constraints. You will need a sound knowledge of both The Mental Health Act and The Mental Capacity Act and be able to think laterally to meet individual's needs.

Job responsibilities

This is a challenging and diverse role, applicants should be confident in their ability to work in demanding situations with good communication skills including diplomacy, negotiating skills and the ability to provide constructive feedback.

There will be a requirement to travel out of county to undertake commissioner oversight visits to provide assurance to the ICB that those individuals with LD/A who may be detained are receiving safe and appropriate care and a discharge pathway is identified. The job holder will work with our system partners and care providers identifying risk, service improvement and governance issues.

Main Duties and Responsibilities

To coordinate and arrange the delivery of the Assuring transformation Cohorts Care (Education) and treatment reviews to the timescale set out by NHSE.

To work in collaboration with community services to develop and implement the Dynamic Support register for the Assuring Transformation cohort.

To ensure that the Dynamic Support Register is regularly reviewed with system partners to ensure that the register accurately reflects those who may be at risk of admission.

To chair community and as inpatient C(E)TRs reviews for individuals with a learning disability and/or Autism as the commissioning representative.

To co-chair, where requested, inpatient C(E)TRs with the Provider Collaborative.

To ensure the accurate completion the key lines of enquiry template following the C(E)TR to in line with NHSE policy and guidance.

To disseminate the action plan from the C(E)TR to community care coordinators and follow up that actions have been completed to timescale.

To utilise the Local Emergency Area Protocol (LEAP) procedure for individuals who are at imminent risk of admission to inpatient settings at the request of the community services and chair these meetings.

To provide support to the community services for ongoing case management where the emergency protocol has been utilized.

To work collaboratively with the local authorities and system partners to enable timely person-centred discharges.

To complete quality assurance visits for all individuals within BNSSGs cohort in accordance with Learning Disability and or Autism Commissioner Oversight guidance.

To work collaboratively with the Host Commissioner for out of area placements.

To work collaboratively to ensure that the discharge targets set by NHSE are met for the BNSSG area.

To oversee the maintenance of BNSSGs Assuring Transformation database enabling accurate monthly returns to NHSE.

To lead in the delivery of training and development to wider system partners in the C(E)TR processes with CCG colleagues.

The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders.

Participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice and expertise.

Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.

Manage corporate business returns, including planning and workforce returns.

Develop and implement a content management system to ensure information is properly managed and best practice is shared across the team, Directorate and the wider NHS organisation.

Take a lead in the coordination of training & development and recruitment activity across the Team.

Contribute to performance improvement, taking a lead for identified areas where agreed.

Contribute to the strategic planning of projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building in contingency and adjustments as necessary.

Contribute to short-, medium- and long-term business plans, achieving quality outcomes.

Develop policies and procedures in own work function with an impact on the wider organisation, as required.

Contribute to performance improvement, taking a lead for identified areas where agreed.

To provide support to the wider funded care team as required.
