An opportunity has become available for a Registered Learning Disability Nurse to join the Funded Care Team within the BNSSG ICB on a 12-month contract. The applicant should be able to evidence knowledge, skills, and working experience relevant to this patient group. Main duties of the job
As part of the NHS England Care & Treatment Review policy deliver and chair Care, (Education) & Treatment Reviews both in the community and in hospital settings for individuals of all ages. This will include those young people and adults in inpatient settings and those at risk of admission.
You will co-ordinate and chair Local Area Emergency Protocol meetings for individuals that are at imminent risk of admission and support our community teams in delivering change.
You will chair Dynamic Support meetings for individuals that are identified as having escalating risks and there is a need to avoid admission to inpatient units.
You will manage a caseload of individuals that are detained under the mental health act, being responsible for oversight visits to ensure that care delivery is being delivered to the required standard. You will also ensure active discharge planning is incorporated into the care pathway.
You will work with a range of stakeholders to support children and young people with learning disabilities/autistic spectrum disorders / mental health diagnosis.
