Job summary

An opportunity has arisen for a registered nurse with experience of older people/complex physical care needs to join the BNSSG - Continuing Health Care team.

This is an exciting opportunity to develop your assessment skills and gain knowledge in a new field. The role will involve assessing and reviewing clients with regards to their health needs, welfare and provision of care whether in a domestic setting or residential placement. Our team is based in the heart of Bristol, North Somerset and South Gloucestershire covering the BNSSG demographic across all areas. We are currently working a hybrid of remote and face to face but continue to deliver person centred assessments of need and subsequent provision of health care if found eligible under the National Framework for Continuing Healthcare.

Main duties of the job

The job holder will be involved in care coordination as well as highlighting service improvement and governance issues. An ability to work collaboratively with other agencies is essential and experience of working in such a manner is desirable. As this role may involve working with clients within other areas of care, such as learning difficulties or mental health a broad range of knowledge and experience is welcomed. Training will be tailored to your experience and the needs of the service.
