Job summary
The Surrey Continuing Healthcare Team is responsible for ensuring that patients referred for NHS Continuing Healthcare funding receive a high quality assessment of need, ensuring that the patient is at the heart of the assessment, their view and those of their carers' are taken in to account of and that the provided within national timescales.
Responsible for supporting the administration of the Referral process(including Fast Track referrals) and the CHC Co-Ordinator ensuring thatthe appropriate documentation has been provided and quality checked foraccuracy. For arranging appointments for initial and review assessmentsfor clients in respect of their eligibility for NHS Continuing Healthcare andNHS-funded Nursing Care, in a timely manner, and for ensuring that alloutcomes have been issued to referrers and clients/representatives.
Discharge to assess allows patients across Surrey to be discharged freely out of Hospitals to other care environments helping to maintain Hospital flow and therefore reduce strain on the acute system.
Main duties of the job
To act as a first point of contact for clients, their families and carers,general public, acute and community providers, other health and socialcare staff and other external agencies with regard to referrals to facilitatethe referrals process, logging all new referrals in a timely manner andprioritize as agreed by CHC Management.
- Processing discharge to assess referrals
- Setting up Discharge to assess payment packages
- Monitoring of the email account
- Liaising with stake holders and patients with relation to CHC assessments
