Job summary

NHS Sussex Commissioners have an exciting opportunity for an experienced candidate in the role of Programme Manager in the Urgent Care team. This role leads on the transformation of discharge pathways from Brighton and Hove services back into the community and supporting the front door models in local hospitals. This role has been developed with, and sits alongside our colleagues in Adult Social Care. As our system transforms following Covid, the successful candidate will have the opportunity to provide programme leadership and development/oversight of the workstreams that take us through transition into our longer-term outcomes. Ultimately, this will improve the patient experience and promote the best use of our resources to ensure patients get the right support, in the right place at the right time.

When in post, you will develop a detailed knowledge of many principles - including HomeFirst, Discharge to Assess, Urgent Community Response and Therapy-Led pathways. You will have daily contact with a large range of partners - from frontline staff to very senior executives. You will be supported every step of the way and can expect to spend time co-located with partners to ensure we are achieving and delivering high quality outcomes for our population. This includes a strong focus on health inequalities.

Main duties of the job

The successful candidate will have previous experience in leading the management and assurance of complex projects and developing effective business processes which enable successful programme/project delivery. In addition, they are likely to have experience providing Programme Management leadership and support, preferably within the health or social care sector. Experience in both commissioner and service provider organisations would be a significant advantage and the ability to communicate clearly and with confidence is essential.

Job responsibilities

Health and social care system partners in Brighton& Hove (B&H) have developed a programme of work designed the transform community services across health and social care in B&H. The aim of the role is to deliver the transformation programme already agreed amongst system partners, which includes delivering a Home First model, trusted assessment, and greater alignment of services. The complexity of this programme requires a programme management function to plan, co-ordinate and support the delivery of actions across several stakeholders. This role is hosted by NHS Sussex, but will sit across Health and Social Care services, being co-located within both and having access to the relevant systems.

The role will involve working closely with placed based colleagues in B&H to:

Deliver expert Programme Management support to a portfolio of programmes as defined by the Head of Urgent Care.

Act as a critical friend to proactively monitor and support the output of the teams programme to enable effective delivery of agreed objectives

Lead the implementation of the phases of the transformation programme, to include Discharge to Assess (D2A), Trusted assessor model, front and back door services, support the training, and cultural changes required

Maintain excellent communication between all stakeholders and collate feedback and input to ensure Community Transformation programme design reflects the needs and process agreed

The development of effective relationships with managers and key stakeholders in the business area across and outside of the organisation. This will include the negotiation of solutions to deliver objectives with a range of internal and external partners

Support Better Care Fund funded projects

Responsible for identifying and managing issues, risks and identifying mitigations to ensure the programme delivers its objectives within the required timeframes.

Provide timely reporting to system partners to support the delivery of progress against agreed programme milestones. And where necessary, is the basis for escalation.

Support the Head of Urgent Care, NHS Sussex; Assistant Director of Commissioning at Brighton and Hove City Council and the Brighton and Hove Area Director - Sussex Community NHS Foundation Trustin the production of assurance reports to internal and external audiences.

Ensure governance principles are applied to all projects, leading project reviews and analysis and providing detailed updates and assurance against the planned and forecast position

Produce project highlight reports and other project documentation to agreed quality standards and in accordance with agreed project management tools and templates

Ensure changes are raised in accordance with change management processes

Lead the identification of benefits and ensure plans for benefit realisation are agreed with Programme Boards

Ensure all programmes follow agreed standards for programme control

Lead programme progress and performance reviews, ensuring lessons are captured and shared with similar programmes elsewhere

Assist with the scheduling of resources within programmes

Ensure the identification and management of interdependencies

Support the Head of Urgent Care in the continuous improvement of Project and Programme management processes within the team.

  • Educated to masters level or equivalent level of experience of working at a senior level in specialist area.
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
  • Comprehensive experience of project and programme principles, techniques and tools such as Prince 2 and Managing Successful Projects.
  • Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement. Experience and/or knowledge of urgent and emergency care services to aid the commissioning function.
  • Should have an appreciation of the relationship between the NHS England, individual provider and commissioning organisations.

Desirable

  • Evidence of post qualifying and continuing professional development.
