Job summary
The post holder will undertake the role of coordinating and completing the assessments of continuing care and complex health care needs for children and young people within Norfolk and Waveney.
The post holder will present cases to the multi-disciplinary eligibility panel and participate in multi-disciplinary team (MDT) level discussions about cases.
The post holder will promote personalization, including personal health budgets within children and young people's continuing care and complex health care.
Main duties of the job
The post holder will:
Have comprehensive knowledge and experience in assessing and evaluating the needs of children and young people requiring NHS Continuing Care for CYP
Work under the direction of Senior Clinical Lead ensuring development of quality, safe and equitable support packages.
Provide clinical assessment in children and young people's eligibility using the National Framework Tool
Coordinate annual reviews for clients following the Continuing Care Framework timeframes.
Report to the Senior Clinical Lead any safeguarding concerns during assessment/review process.
Provide evidence based holistic assessment and recommendation for the Continuing Care Panel to determine a support package required.
Attend MDT meeting/reviews, liaising with partners to ensure an integrated approach to ongoing support
Support the transition process.
In absence of the senior nurse, attend meetings as requested and feedback to the team.
Support the CYP commissioning team with care, education and treatment reviews (CETRs) for Transforming Care clients, providing advice were necessary.
Identify equipment and consumables for continuing care and complex health care clients, following the equipment pathway process for Norfolk and Waveney ICB.
Use a best outcome and quality mind set when identifying support packages with family so needs are being met in a holistic way.
