Job summary
An exciting opportunity has arisen for an experienced and highly motivated Nurse to work as NHS Continuing Healthcare Nurse/Practitioner for Discharge to Assess Pathway 3.
This post offers the opportunity to enhance assessment, decision-making skills and complex case management skills in a challenging but supportive environment. You will also be a member of the Integrated Discharge Hub. You will be responsible for assisting with the discharge planning, assessment and commissioning of care for individuals from the acute hospital.
We are looking for a registered nurse who is keen on integrated working, an understanding of hospital discharges would be desirable, but support and guidance will be on hand. You will be adaptable, have good communication skills and enjoy working with other professionals. This post is based at James Paget University Hospital , working with the Integrated Discharge Team in the hospital, to complete assessments, attendance on wards required at times. Traveling to visit patients and attend meetings will also be involved as required.
You will offer a person-centred approach to individuals and their families. Working in partnership with other agencies and have a strong commitment to improving the outcomes for those on your caseload. You will have proven skills in working with people who have complex health needs. You will also be able to demonstrate that your practice is evidence based and innovative.
Main duties of the job
- To jointly assess with other health and social care colleagues, patients ready for discharge from hospital to identify who will be accountable for case managing whilst on the Discharge to Assess Pathway.
- The post holder will take responsibility for collaborative working practices to deliver the D2A3 (complex discharge) Pathway supported by the Band 8a Senior Nurse.
- The post holder will work collaboratively with the acute hospital patient facing services to support successful discharge on a personalised patient pathway.
- Where required will review with the Senior Nurse to establish an effective interface between D2A3 and existing hospital discharge pathways.
- Ensure high quality, safe and timely patient flow from the hospital through to discharge within the community.
- Ensure the accurate application and quality assurance of decision making, ensuring patients have access to the full range of available and appropriate discharge pathways.
- Provide effective communication with patients and their families to ensure engagement and understanding of the D2A3 Pathway and potential onward transfer and support.
