We have an exciting opportunity to join the North Norfolk Place Team, working in a diverse role supporting integrated working within an embedded system. You will be working closely with Practice Managers, Clinical Directors and system leaders at all levels to support development and provide assurance on delivery across a broad range of areas.

Main duties of the job
We are looking for a highly motivated, forward-thinking individual to join the North Norfolk Place team.
You will have excellent relationship building, data analysis, contract management and project management skills, along with a positive attitude.
The focus of the role will be:
- Contribute to the strategic planning and resilience of local services, which respond to the needs of the local population and through direct collaboration with general practice.
- Support the transformation and delivery of health and care programmes across the Place, by taking a lead role to develop and deliver activities which meet national, ICS and locality strategic objectives.
- Working closely with Primary Care Networks and other system partners to build Integrated Neighbourhood Teams
- Taking a population health management approach to identifying and addressing local needs
- Building relationships with partners to develop influence both within and outside of the ICB
- Leading and being responsible for multiagency projects that promote integrated care
