Job summary

We have an exciting opportunity to join the South Norfolk Locality Team, working in a diverse role supporting integrated working within an embedded system. You will be working closely with Practice Managers, Clinical Directors and system leaders at all levels to support development and provide assurance on delivery across a broad range of areas.

Main duties of the job

A comprehensive knowledge of general practice, the local health and social care landscape and our local communities together with a proven track record of fostering and maintaining productive working relationships, alongside an ability to respond to day-to-day operational pressures whilst managing multiple projects spanning across multiple organisations will all be key for the successful candidate. Previous knowledge of the PCN DES and a good understanding of PCN Development is essential.

You will need to be able to receive complex information, digest, prepare and share relevant points with a range of stakeholders as well as respond and offer solutions to a wide range of queries which come into the team.

The ability to support effective partnership working, often within tight timescale in a pressurised environment where there can be multiple and completing demands will also be key to balance the need to deliver transformational change with day-to-day operational pressures.

You will need to demonstrate your track record in building excellent relationships, the ability to work autonomously as well as sound judgement when dealing with competing priorities and tight timescales.

Job description

Job responsibilities

  • To support the development of primary care networks to become fit for purpose to build resilient primary care that can deliver the ambitions of national and local strategies.
  • To work with the PCN Development team, GPPO, PCNs and system partners to support redesign, commissioning and implementation activities within the PCN areas.
  • To support practices with day to day operational issues and work with colleagues across all departments to find appropriate solutions.
  • Identify needs around key enablers including workforce, estates and resilience and work with the relevant teams in the offices of the AD of Primary Care and the CFO to establish appropriate solutions.
  • Working in the context of the wider team and the strategic commissioning team support the delivery of a programme of GP Visits and educational events.
  • Deliver a comprehensive programme of communication and engagement, to build strong relationships and mutual understanding.
  • To support the management and implementation of development initiatives, supporting service redesign, QIPP delivery and integration.
  • The post holder will be expected to support all aspects of primary caredevelopment and transformation.
  • You will be required to interface and liaise with a range of organisations including for example; GP Networks, NHS England, local Acute and community colleagues, patients and Social care and voluntary organisations
  • The post holder will have a particular focus on supporting the GP Network and assist in putting plans in place to improve primary care access/transformation.
  • To contribute to the strategic planning and development of practices/services to respond to the needs of the practice populations
  • To develop and maintain positive credible relationships with key partners within the network, fostering a culture of collaboration for the delivery of equitable, high quality care.
  • Responsible for drafting changes, implementation and interpretation to policies, guidelines and service level agreements (SLAs)
  • Initiate and update reports including a risk and issue register
  • Promote, develop and enable effective communication across primary care, other PCN Networks and the wider system
  • Act as lead for projects identified by the PCN and endorsed by the Clinical Director
  • Act as key point of liaison internally and externally for matters relating to general practice ensuring a timely response.
