Nurse Assessor/Case Officer

Job summary

An exciting opportunity is available to join the all age continuing care service covering the North East and North Cumbria as a children's continuing care (CCC) nurse assessor/case officer to potentially to cover the North Cumbria locality. The service undertakes assessments and commissions packages of care across the North East and North Cumbria in line with the National Frameworks for Continuing Healthcare and Children's Continuing Care. Main duties of the job

The CCC nurse assessor/case officer is a key role in the CCC service meeting the unmet health needs of children and young people in the NENC area. The CCC team are keen to hear from qualified nurses with experience of undertaking assessments for children and young people with a variety of health needs especially mental health and or learning disability.

Building and maintain relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector Contributing to the ICB's achievement of their statutory duties. Supporting the transition of young people to adulthood. Contributing to the ICB statutory duties for SEND.

  • First Level Registration NMC - RSCN, degree or relevant experience

Desirable

  • Qualification in children's health care or Learning Disabilities

  • Previous experience of working within an extremely busy office environment
  • Experience in working within a multi-disciplinary team setting
