We currently have a number of exciting opportunities to join the All Age Continuing Care Nursing and Case Management teams These posts cover all of NHS continuing care including adults, children and complex case management.
There are a variety of positions including case management and nurse assessor positions at band 6. Roles will be ICB wide covering North and South of the geography, with bases at the nearest ICB base to home.

Main duties of the job
These roles will be fast paced and interactive with patients, families, clinicians and other partner agencies so will require good communication skills, attention to detail and teamwork approach.

Our Integrated Care Board (ICB) is the largest in the country.

The role of the post holder is:
- To Chair and lead multi-disciplinary teams in the completion of Decision Support Tools to apply the NHS National Framework for Continuing Health care and Funded Nursing Care.
- To undertake reviews in line with the National Framework for Continuing Healthcare and Funded Nursing Care
- To process Local and Independent requests for appeal against NHS Continuing Healthcare eligibility decisions for the ICB
- To undertake reviews for Previously Unassessed Periods of Care (PUPOC) on behalf of the ICB.
- To undertake Retrospective reviews on behalf of the ICB.
- Complete the Appeals process within set timescales.
- Completion of the individuals Care Needs Portrayals in order to inform the Appeal process.
- Respond to NHS England data requests in a timely manner.
- Arrange and facilitate Local Appeal meetings & Panels.
- Ensure a patient focused approach in identifying health needs and facilitating choice and control (for example via Personal Health Budgets) inthe most appropriate and cost-effective care pathway for individual patients.
- To be involved in the ongoing development of resources and expertise within the Continuing HealthCare Department.
- To support the seamless implementation of the NHS National Framework for Continuing Healthcare and NHS Funded Nursing care including contribution to the development of training programmes and delivery plans as required to facilitate others learning.
- A strong business focus as well as clinical expertise is a key requirement for this role to ensure delivery of best possible care within available funding representing "value for money" within the ICB Choice and Equity Policy.
