An exciting opportunity is available to join the all age continuing care service covering the North East and North Cumbria as a children's continuing care nurse (CCC) specialist. The service undertakes assessments and commissions packages of care across the North East and North Cumbria in line with the National Frameworks for Continuing Healthcare and Children's Continuing Care. Main duties of the job

The children's continuing care nurse specialist is key in supporting the CCC service to achieve the key performance indicators and deputising for the CCC Lead when required. The CCC team are keen to hear from qualified nurses with experience of managing a team, of assessing and improving the health outcomes of children and young people plus evidence of working in partnership with a wide range of stakeholders. This role will be responsible for:

o Monitoring and quality assurance of the nurse assessor's performance.o Supervision of staffo Supporting the commissioning of packages of care to meet the unmet health needs of children and young people eligible for CCCo Data monitoring for KPI'so Clinical expertise in the health needs of children and young people.o Engagement with key stakeholderso Building and maintain relationships with Local Authorities, providers, Foundation Trusts and the voluntary sectoro Contributing to ICB's achievement of their statutory duties.o Ensuring the ICB meet their statutory duty for SEND.

  • Registered Nurse qualification - qualified paediatric nurse with experience of complex paediatric nursing; Registered sick children's nurse or learning disabilities nurse - degree level or working towards with extensive post registration experience at level band 6 or above, working with children with complex and life limiting illnesses.

Desirable

  • Qualification in children's health care or Learning Disabilities. Management
  • district nursing (BSc, Diploma or Certificate) Health Visiting SCPHN
  • Non-medical Prescribing Certificate, etc.

Experience

Essential

  • Experience in working within a multi-disciplinary team setting
  • Budget Management Experience
  • People management experience
  • Experience of reviewing individual's health and nursing care needs
  • Experience of completing specialist nursing care assessments
  • Experience of applying the CC eligibility process to individuals
  • Relevant management experience of working in a complex organisation preferably NHS
  • Experience of managing complaints and disputes
