An exciting opportunity awaits the right candidate as NHS North East and North Cumbria Integrated Care Board (ICB) seeks to recruit an experienced and self-motivated individual to join the Children and Young People Commissioning Team in Tees Valley as a Special Educational Needs and Disabilities (SEND) Health Facilitator. Working in a busy, high-volume but friendly and supportive environment, this role will suit an organised person with an eye for detail and plenty of team spirit .By joining our team in Tees Valley Integrated Care Partnership (ICP) you will be supporting the continued development of high-quality health provision to improve health outcomes and reduce inequalities for the Children and Young People of Hartlepool, Stockton-on-Tees, Darlington, Middlesbrough and Redcar and Cleveland. The Special Educational Needs and Disabilities (SEND) Health Facilitator is a key role in supporting the Designated Clinical Officers to ensure delivery of our objectives outlined in the Children's and Families Act (2014) and the SEND Code of Practice (2015) and will be responsible for supporting children, young people and their families to navigate and receive the health services they need at the earliest possible opportunity.
Main duties of the job
The Special Educational Needs and Disabilities (SEND) Health Facilitator will work as part of the Children and Young People Portfolio to commission high quality health services for patients across Tees Valley.
The post holder will be responsible for:
Supporting the Designated Clinical Officers in providing leadership and direction on Special Educational Needs and Disability (SEND) delivery in the Tees Valley for 0 to 25-year-olds taking account of national and local strategy.
Represent the ICB at regional and national network events
To participate in whole systems service quality improvement programmes as required including SEND improvement plans and to always practice within a framework that promotes quality and positive family experience.
Make a positive contribution to excellent communications across the system, ensuring effective communication with staff and stakeholder involvement whilst supporting the development of effective partnership working with all partner organisations.
To always promote equality in the commissioning of care and support for children and young people.

- Also hold a professional qualification: Registered Nurse: RNC / RNLD
- Or be a Registered Allied Health Professional e.g., Occupational Therapist, Speech & Language Therapist, Physiotherapist or Specialist Public Health Practitioner
