and funded solely by the NHS where the individual has been found to have a
Primary Health Need as set out in the National Framework for NHS Continuing

team at Barnsley Place however as part of NHS South Yorkshire Integrated Care

Main duties of the job
Applicants must hold a valid NMC registration and show
Continuous Professional Development within their application form.
The main duty of the role is facilitating Continuing
Healthcare/Funded Nursing Care assessments and reviews for patients
This will contribute to the maintenance of the
appropriate level of funding support.
Ensuring there is an effective, up to date patient
care management plan which meets the requirements of the individual and is
reviewed as per Barnsley CHC processes.
Co-ordination of the assessment process and where
necessary supporting and writing key elements of the patient care plan to
reflect presenting health needs.
Work closely with our health and social care
partners in the best interest of the patient.
The individual must be able to work autonomously but
also be active part of the team maintaining the values and beliefs of NHS South
Yorkshire ICB at all time.

The CHC Nurse Assessor
will work with relevant health professionals, CHC colleagues, business support
staff, senior leads and other support staff within the CHC team to ensure
delivery of key targets and products required.
They will undertake or
manage timely review of all Continuing Healthcare/Funded Nursing Care patients
functioning as a central point for referral, assessment and support.
This will contribute to
