have this fantastic opportunity to recruit an additional team member working 30 hours per week.
Our CHC Nurse
Assessors have an important function to ensure that each element of our CHC
spend is clinically justified by using the CHC Decision Support Tool to
determine the level of care needs of individuals. Well expect you to
demonstrate knowledge, skills and experience including:
- A degree or equivalent experience working at a similar level in a specialist area
- NMC registration required
- Good understanding of the health & social
care environment
- Motivation to learn and constantly improve processes
and outcomes
- Effective problem-solving skills
- Hold a UK driving licence with access of own car
for work purposes.
Main duties of the job
Applicants must hold a valid NMC registration and show Continuous Professional
Development within their application form.
The main duties of the role is facilitating
Continuing Healthcare / Funded Nursing Care assessments and reviews for
patients
This will
contribute to the maintenance of the appropriate level of funding support and
ensure quality care is in place based
on individual patient needs.Commissioning
of appropriate continuing healthcare funded packages of care to meet the
identified care needs of individual patients deemed eligible. Ensuring that they
adhere to the Rotherham Commissioning Principles.

Ensuring there is an effective, up
to date patient care management plan which meets the requirements of the

Co-ordination
of the assessment process and where necessary supporting and writing key
elements of the patient care plan to reflect presenting health needs and how these
will be supported.
Work closely with our health and social care partners in the best interest
of the patient.
