Job summary

Due to a recent retirement, South Yorkshire Integrated Care Board (SYICB) Sheffield Place has an exciting opportunity available to join the senior team within NHS Continuing Healthcare.We require an experienced registered nurse with leadership experience to lead a team of dynamic and progressive CHC nurses.

The Sheffield Continuing Health Care team are an innovative and friendly team who deliver a service to the population of Sheffield.This is an exciting opportunity to be involved in a team, who are working closely with our Local Authority colleagues and our wider South Yorkshire partners, towards a more integrated approach in how we assess and meet an individuals care needs.

NHS continuing healthcare (CHC) is a package of ongoing care that is arranged and funded solely by the NHS where the individual has been found to have a Primary Health Need as set out in the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care. The post holder will be responsible for leading a team of CHC nurses including managing recruitment and selection, personal development reviews and development planning, managing performance and sickness absence and ensuring appropriate support and effective clinical supervision is provided.

Main duties of the job

The post holder will have an expert knowledge of NHS Continuing Healthcare and will provide guidance and expertise to staff carrying out assessments according to the framework ensuring that service needs are met in a timely manner.

Job responsibilities

The post holder will provide clinical input into the assessment and care management process for CHC as part of a wider management team and will support the Operational Lead and CHC team to ensure that work programs are planned, managed effectively and successfully delivered.

The post holder will be responsible for providing clinical support including facilitating assessments and reviews against the appropriate frameworks and policies and ensuring the team present documentation for panel assessments. This includes providing and receiving highly complex, sensitive and contentious information.
