Job summary

South Yorkshire Integrated Care Board are seeking and Portfolio Lead Transformation and Delivery Health and Care to lead one of four localities within Sheffield Place.

Working as part of the Place Team the locality leads are responsible for supporting the 74 general practice and 15 primary care networks within Sheffield.

The post holder will work with other team members to provide management and strategic leadership for the locality, and general practices within the locality/ Primary Care Networks.

The post will act as a conduit in shaping and delivery of responsibilities with the ICB and system partners.

They will support, enable and encourage general practices to become engaged providers and to contribute to the Health and Care Partnership and SYICB priorities.

Main duties of the job

Providing management and strategic leadership for the locality, and its locality member practices / Primary Care Networks and supporting, enabling and encouraging practices to be engaged with Sheffield Place, SYICB and system partners, the postholder will:

Support GP and clinical engagement within the localities and Primary Care Networks

Shape and lead the strategic direction of the locality.

Lead the development and implementation of locality strategies, the annual business plan.

Provide leadership within the locality encouraging innovation within general practice.

Contribute to the development and implementation of the South Yorkshire Primary Care Strategy in Sheffield and the Health and Wellbeing Board priorities for Sheffield.

Encourage effective engagement by practices and PCNs in development and delivery of integrated health and social care services.

  • Educated to Masters level or equivalent in a relevant area of academic practice or equivalent experience working at a commensurate band/level.
  • Additional management qualification or specialist qualification at post-graduate or equivalent level of experience.

Desirable

  • Managing Successful Programmes Practitioner
  • Managing Successful Programmes Advanced Practitioner
  • P3O Practitioner
