Job summary

A NUMBER OF IPA PRACTITIONER ROLES AVAILABLE FOR IPA TEAM WITHIN LANCASHIRE & SOUTH CUMBRIA

ROLE IS FAMILY FRIENDLY AND OPEN TO ALL ALLIED HEALTH PROFESSIONALS & NURSES

The post holder will play a key role in supporting the IPA service across the region.

Working within the remit of the IPA & Joint Funding, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures

The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

Main duties of the job

Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for Joint Funding, S117 Funding & Learning Disability & Autism package funding

The IPA Team within the ICB aims is to improve the health of the people and reduce health inequalities through strong, clinically-led commissioning of high quality healthcare services.

An exciting opportunity has arisen within our Individual Patient Activity Team. Role involves assessing patients within IPA funding streams, holding a caseload and reviewing patients as necessary. Good induction and training offered. Friendly Multi-disciplinary Team. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers

You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work.
