We are seeking a Senior Lead for Strategic Place Delivery who will lead on integrated delivery of the full range of community services in the South Cumbria place, ensuring identification and implementation of opportunities for improvements in quality, efficiency, and outcomes, and delivery of key standards/targets and quality requirements. This will include ensuring that operational teams work together across different organisations / sectors to deliver integrated services.
Passionate about improving care and outcomes, you will have significant senior leadership experience in a relevant sector, with demonstrable experience of delivering resident-focused services that are integrated and co-designed. You will combine strengths in relationship building and compassionate leadership with the ability to review and transform current service models.

Main duties of the job
The role will be varied and will adapt over time as the ICB develops. However, the post-holder is expected to fulfil the following key functions:
1. Identify and maximise the opportunities for integrated delivery of community services in the South Cumbria place.
2. Lead on the creation of operational plans and ways of working to ensure delivery of strategic aspirations and required standards/targets

The South Cumbria place is committed to putting our residents at the heart of what we do, listening to people with lived experience, understanding their needs and co-designing solutions that work best for our communities. A geographically large and diverse place, we support residents across coastal and rural areas, with a mixture of affluent and highly deprived populations. Unlike other places in our system, South Cumbria is not coterminous with any one local authority, but instead works with three unitary councils which were formed in April 2023 - Westmorland and Furness, Cumberland, and North Yorkshire. This provides significant opportunities for new ways of working.

Opportunities for integrated delivery of community services in the South Cumbria place1. Lead the operational coordination and transformation of integrated community services across the South Cumbria place. This will include community nursing and therapies (all age) across physical and mental health, learning disabilities and autism.2. Ensure all providers of community services are identified (including those by the voluntary, community, faith and social enterprise sector) and that their individual and collective roles, responsibilities and service specifications within the South Cumbria place are clearly defined.3. Use a population health management approach, coupled with listening to the voice of to our residents, to understand the needs of residents and ensure that the full range of community services are provided in a way that meets these needs whilst ensuring financial sustainability.4. Identify opportunities for improvements in quality, efficiency, and outcomes for residents across the full range of community service provision, including: Community service provision into Integrated Care Communities (all age) Community nursing and therapies (all age) across physical and mental health, learning disabilities and autism Pharmacy / medicines management Falls service Community stroke services Discharge to Assess 2-hour urgent community response Virtual wards Mental health initial response service Continuing Health Care Care market sustainability5. Identify and implement innovative, integrated solutions to keeping people well at home, ensuring unnecessary hospital admissions are deflected and discharge from hospital is timely and safe, actively participating in initiatives such as the establishment of a Wellness Centre and the Whole System Flow programme.6. Proactively seek opportunities for shared roles and training opportunities across community service provision.7. Identify areas of good practice from elsewhere that can be successfullyimplemented in the South Cumbria place.8. Establish and maintain connectivity to and alignment with system-widetransformation programmes across the Lancashire and South Cumbria footprint, understanding how the requirements of the place will be delivered through these programmes and/or how system-wide activities may impact upon the place.Operational planning to ensure delivery of strategic aspirations and required operational standards/targets1. Ensure that operational plans are in place across providers to support delivery of key standards/targets and quality requirements that the place is responsible for delivering. As a minimum, this will include: 2-hour Urgent Community Response Delayed Transfer of Care / No Medical Criteria to Reside Access to Improving Access to Psychological Therapies (IAPT) and mentalhealth support for children and young people Unplanned hospital admissions for chronic ambulatory care conditions Older people at home 91 days after discharge from hospital intoreablement/rehabilitation services Length of stay in hospital, measured by % of inpatients with a stay for longer than 21 days Emergency admissions due to falls2. Lead on the connectivity to and alignment with operational planning activities across the relevant directorates of the ICB.3. Coordinate the operational resources deployed to place from the ICB, ensuring that delivery in place meets national and local requirements.4. Ensure operational teams work together in a one team approach across different organisations / sectors to deliver integrated services.5. Deliver measurable improvements in key operational standards/targets and quality requirements through actively engaging stakeholders in a more integrated approach to health and care delivery.
